Apply for Reinstatement

All Reinstatement Applications are reviewed by the CPE and Licensing Committees, which can cause a delay in processing the application.

Inactive Registrants who desire to reinstate to an active permit to practice must complete the following:

  1. Complete 120 hours of approved continuing education (including 4 hours of ethics) within the three calendar years prior to when you wish to reinstate to active status. Documentation of successful completion will need to be sent to the Board.
  2. Application to Reinstate to Active Permit

**Please note – CPE hours used for reinstatement CANNOT be used to renew your permit. Depending on your renewal period, you may need additional hours to renew. Please contact the Board office for clarification.
